Coast Guard flight name tags
Coast Guard aviators train with the Navy and wear the same gold wings. Their tags follow suit: black leather, gold letters.

What the uniform manuals say, and what people wear
The Coast Guard uniform manual (COMDTINST M1020.6K) does not describe a flight name tag; we searched the whole manual. Its name tags are plastic, for the dress uniforms (section 4.C); flight clothing is left to the Aviation Life Support Equipment Manual (section 5.D.6), and the G-1 leather jacket is limited to aviation personnel (section 5.D.11). So the style comes from practice. Among the tags we measured it is almost always gold on black, either four boxes or the full name on top with rank left and USCG right.
Layouts
Four boxes and name-on-top are both common on Coast Guard tags; pick whichever your air station wears.

Colors
Gold on black is the norm. Silver and the brown options are available.
Wings
Aviator (the gold Naval Aviator insignia Coast Guard pilots earn), Flight Officer and Aircrew wings. The wing art is drawn from the official insignia and engraved at the same size on every tag, about two-thirds of the tag width, like the tags we measured.
